With CCM, you receive support between visits to your healthcare provider. Our team—comprising your primary care provider, a case care manager and other staff—will regularly monitor your health and help you manage your chronic conditions through personalized care plans, routine check-ins, and guidance on medications, treatments, and lifestyle changes.
3. What are the benefits of joining the CCM program?

4. Who is eligible for CCM?
To be eligible for CCM, you must two or more chronic health conditions that are expected to last at least 12 months. Common conditions include hypertension, diabetes, arthritis, Ehlers-Danlos syndrome, and depression, among others.
5. What services are included in CCM?
Our CCM program includes the following services:
Regular monthly check-ins from our care team
Medication management and reconciliation
Help coordinating care between different healthcare providers
Assistance in creating and updating your personalized care plan
24/7 access to a member of our team for urgent health concerns
6. Is there a cost associated with CCM?
Insurance covers most of the cost for CCM services, but there may be a small copayment or deductible depending on your insurance plan. We recommend checking with insurance or your supplemental insurance provider to understand any out-of-pocket costs.
7. How often will I hear from the CCM team?
You will have at least one monthly check-in with our CCM team. Additional check-ins are available as needed, based on your health status and care needs. You also have the option to reach out to our team anytime with questions or concerns.
8. Can I still participate in CCM if I see other specialists?
Absolutely! Our team collaborates with your other healthcare providers, including specialists, to ensure all aspects of your care are aligned and that you receive comprehensive, coordinated support.
9. Do I have to come into the office for CCM services?
No, CCM services are mostly provided over the phone or through secure messaging. This allows us to support you even between in-person visits.
10. How do I sign up for CCM?
Signing up is easy! Simply let our front desk staff or your primary care provider know that you're interested in CCM or enroll on our website.
11. What if I want to leave the CCM program?
You can choose to stop CCM services at any time. Just let us know, and we’ll discontinue the program without any penalties or changes to your regular care.
